Applications MUST be filled in triplicate on PSC Form 3 (Revised 2008), and ESC Form 3 (Revised 1998) for Education Assistant II, each with attachment of photocopies of required certified Pass slips, academic transcripts, passport size photographs (coloured), identity cards indicating job tittle and reference numbers
